Cancellations & No-Shows
Your appointment must be cancelled at least 90 minutes prior to the arrival time to avoid a fee of $25.00

Yearly Fees
The yearly fee is $84.00 for each patient and is due each year on May 1st. 

Return-Check Fee
A $25.00 fee is applied for any check that is returned.

Late Arrivals
Any patient more than 20 minutes late may need to be rescheduled or scheduled with a different provider. 

Records Fee
A records fee may apply if your medical records are requested to be released more than once.

Controlled Substance Prescriptions
For patients on controlled drugs, you are charged $15.00 each time you request a medication refill electronically.

Paper prescriptions that are picked up at the office are $10.00 each time.
